Community navigation for health-related social needs

We support people who need help with food, benefits, transportation, housing resources, healthcare access, and everyday administrative barriers that affect health and stability.

Support with public benefit questions, applications, document readiness, and referrals to appropriate community programs.

Food & basic needs coordination

Connection to food pantries, nutrition resources, emergency assistance, and community-based support networks.

Help identifying barriers such as transportation, appointment coordination, health insurance questions, and follow-up needs.
