We support people who need help with food, benefits, transportation, housing resources, healthcare access, and everyday administrative barriers that affect health and stability.
Food & basic needs coordination
Connection to food pantries, nutrition resources, emergency assistance, and community-based support networks.
Health access support
Help identifying barriers such as transportation, appointment coordination, health insurance questions, and follow-up needs.

This private preliminary screening helps us understand your needs and prepare for a navigation appointment. It is not a Medicaid eligibility determination and does not guarantee services.
